Financial InfoMint Physical Therapy is a cash-based private practice. Here's why:

Insurance companies dictate how many physical therapy treatments are allowed/must happen in a certain window of time. You can have too many, or too few treatments in that window. Usually, the minimum is 1 physical therapy treatment per week. Any less than that is considered "not medically necessary" and the physical therapy claim is denied and the patient is responsible for payment.

I treat my patients one time every 3 weeks or beyond. Insurance companies don't like that and won't pay.

This means I do not accept insurance. I am out-of-network with all insurance companies. I do not accept Medicare or Medicaid.

I can accept HSA/FSA.

I can provide a superbill*, which is a document with ICD-10 diagnosis codes and PT procedure codes that you can submit to your insurance for possible reimbursement. It is your responsibility to know if you have met your deductible or if my services are eligible for reimbursement with your insurance company. I will provide any documentation required for reimbursement.

*I cannot provide a superbill for you to submit to Medicare or Medicaid.

I will not contact your insurance company, representative, or union on your behalf.
